27. Surgical Indications for Non-rhegmatogenous
Retinal Detachment

Purpose: To discuss surgical intervention in exudative or serous retinal detachment. Surgical intervention remains controversial and is usually deferred, delayed or not offered to patients with exudative retinal detachment. A review of the literature and a series of cases will be discussed. Indications for surgical intervention and possible techniques of intervention will be discussed.
Methods: A retrospective review of cases of exudative retinal detachment and their surgical intervention was undertaken. All patients despite attempts at treatment of the underlying etiology had visual deterioration. Four cases were identified. All patients were managed with vitrectomy. Final retinal reattachment and final visual acuity were reported.
Results: A systemic work-up for inflammatory, infectious and neoplastic causes of non-rhegmatogenous retinal detachment was undertaken in all four patients. All patients had improved visual acuity following surgical intervention. One patient despite visual improvement had a persistent inferior retinal detachment. Although early post-operative visual acuity had improved compared to pre-operative vision, one case resulted in a further decline in vision.
Conclusion: Surgical reattachment in non-rhegmatogenous retinal detachment may be an important treatment consideration. Timely intervention may help spare visual deterioration in macula-involving serous detachments of unknown etiology and in known etiologies where medical management has failed.
