8. Management of retinal redetachment without PVR

The indication of pars plana vitrectomy in retinal detachment surgery is expanding constantly.
Howver, its role for the treatment of uncomplicated recurrent retinal detachments has not yet
been defined clearly.
The author reports his experience with buckle revision, vitrectomy alone or both in combination
for the management of such cases. 65 patients were retrospectively reviewed. 11 underwent
vitrectomy and endotamponade only (Group I), 43 underwent buckle revision (Group II)
and 11 patients underwent a combined approach (Group III). The analysis showed that anatomic
success rates are comparable, but complication spectra among the groups are differing.
Medical and non medical pro- and con-arguments for each method are presented.
At present the advantage of vitrectomy in “simple” redetachment surgery is not proven and the
surgeons experience must influence the decision.
