45. Comparision of transconcuntival (23 g, 25 g) vitrectomy systems

Objective: To evaluate and compare the safety, feasibility and usefulness of 25-guage, 23 Gauge
and Intrector transconjunctival sutureless vitrectomy systems.
Design: Retrospective review of a consecutive interventional case series. Participants: 21 patients, including diabetic vitreous haemorrhage ; BRVO with vitreous haemorrhage ; Eales disease and ERM patients, with follow-up of 6 weeks.
Intervention: Patients undergoing surgery were randomized between 23G and 25G TSV. Separate patient series was used for Intrector. Main outcome measures: Operative time; visual acuity; wound integrity and complications.
Results: The average operative time was 49 min with 25G, 42 min. with 23 G. All patients had stable wounds. At present data available for 25G (21 patients): pre-operative visual acuity was better than 6/36 in 2 patients (10.5 %); between 3/60 – 6/60 in 4 patients (21.1%); and between hand movements – 2/60 in 13 patients (68.4 %). Postop, 9 patients (52.6 %) had visual acuity between 6/6-6/12; 3 patients (15.7 %) between 6/18-6/36; 3 patients (15.7 %) between 6/60-3/60; 4 patients between 2/60 hand movements. The mean follow-up was 6 weeks. 3 patients developed recurrent vitreous haemorrhage.
Conclusion: TSV systems seem to be practical and safe for selected vitreo-retinal procedures. They reduce operative time, max with 23 G and probably Intrector ; post-op inflammation and have good results in selected vitreoretinal diseases.
