31. Indications for Multiple Therapy with the Intrector®

The Intrector® (Insight Instruments, Inc.) is a true office or operating room portable vitrectomy
instrument for simultaneous vitrectomy and injection of fluids, drugs, air, etc. One can perform
biopsies, open up an after cataract, remove precise volumes of the vitreous and substitute
by BSS, drugs (e.g. in age related macular degeneration = ARMD), air or gas (e.g. pneumatic
retinopexy), or vitrectomize an eye e.g. suffering from a diabetic retinopathy with macular
edema and/or bleedings. The effects of a multiple therapy (PDT, core pars plana vitrectomy =
cPPV, Dexamethason, Avastin) on chorioidal neovascularizations in ARMD were studied prospectively
and are summarized with a follow up of about 14 months.
This true 23 gauge transconjunctival approach allows to avoid any leakage of fluid/drugs or
vitreous prolaps through the injection site.
The core pars plana vitrectomy (cPPV) is considered to induce vitreous liquifaction. As a consequence, more oxygen – a
potent anti-VEGF – is being provided to the retina.The approach will be explained and the setup in the office/operating room
discussed.
