20. Anatomic outcome of internal limiting membrane peeling
and in-flushing in idiopathic macular hole surgery

Purpose: To investigate the anatomic results of vitrectomy with non-stained ILM peeling and
adjunctive hole in-flushing for idiopathic macular hole.
Methods: 20 eyes in 19 patients were included in a retrospective consecutive case series
study. Three eyes were in stage II, 11 eyes in stages ?, and 6 eyes in stage ?. Minimum diameters
of the hole ranged from 323 to 1100μm, mean 532μm as measured on optical coherence
tomography (OCT). Surgical technique included pars plana vitrectomy with ILM +/- epiretinal
membrane peeling (5 eyes) without any staining plus triple in-flushing the hole’s cavity using
flute-needle. Hexafluoroethane (C2F6) tamponade was used in 16 eyes and SF6 in 4 eyes with
7~10 days of face-down positioning. Follow-ups were 3~14 (mean, 5.8) months.
Results: Macular hole closure following one surgical procedure was in 18 of the 20 eyes, and different extent of post-op
visual improvement in all successful eyes. No post-op complications were observed.
Conclusions: Vitrectomy with non-stained ILM peeling and adjunctive hole in-flushing may bring favorable anatomical
outcomes for idiopathic macular hole.
