TRAUMA OF THE POSTERIOR EYE SEGMENT AND VITREORETINAL SURGERY

Puropse: To evaluate the visual and anatomical results of serious mechanical injuries of the posterior eye segment.

Methods: A retrospective noncomparative case series of 82 patients treated surgically for mechanical injuries (except of injuries with IOFB) damaging the posterior segment od the eye in our Separtment in years 1996 - 2000. The initial pathological intraocular changes, surgical attempts, final anatomical and fucntional results are evaluated. The US Eye Injury Registry Ocular trauma score will be presented and also tested in a small group of our patients.

Results: Three eyes with globe rupture were primarily eviscerated and two eyes after the primary repair of the lacerating wound were later enucleated. In 77 patients was performed pars plana vitrectomy. A good anatomical result was achieved in 78,9 % of the eyes with contusion, in 61,5 % with perforating injury, in 60,0 % with penetrating injuriy without IOFB and in 24 % of the eyes with globe rupture. The final visual acuity better than 6/36 was achieved in 52,6 % of the eyes with contusion, 28 % with penetrating injury, 23 % with perforating injury and only in 12 % of the eyes of the globe rupture.

Conclusions: The prognosis in mechanical injuries of the posterior eye segment depends mostly from the initial pathological changes of the intraocular structures. The proper microsurgical treatment enables to save at least some of these severe injured eyes.
