EXOGENOUS AND ENDOGENOUS FUNGAL ENDOPHTHALMITIS

Purpose: The purpose of this paper is to present our results in the management of fungal endophthalmitis.

Methods: Five patients with clinical picture of endogenous fungal endophthalmitis and one patient with exogenous fungal endophthalmitis were treated with pars plana vitrectomy combined with Amphothericin B intrevitreous injection. Systemic anti-fungal therapy was additionally applied in the most severe cases.

Results: The diagnosis of Candida endogenous endophthalmitis in four patients, Aspergillus endogenous endophthalmitis in one patient and Candida exogenous endophthalmitis in one patient was made by obtaining the vitreous material during the surgery for microscopic evaluation and culture. Good anatomical and functional results were obtained in four of six treated patients.

Conclusions: Pars plana vitrectomy in the management of fungal endophthalmitis makes it possible to collect material for culture, removal of viable organisms and inflammatory products, and drug administration. We strongly recommend silicone oil tamponade during first procedure in the most severe cases of fungal endophthalmitis.
