Purpose: To assess how vitreous changes can influence features, evolution, choice of surgical approach and postoperative outcomes in retinal detachment (RD) with macular hole (MH) in high myopic eyes.

Methods: One-hundred seventy eight (178) eyes with a myopia between - 14 and - 30 diopters (d) belonging to 160 patients (mean age 45.2 ± 9.54 years) with a diagnosis of RD due to MH were included in the study. The eyes were divided into two groups on the basis of vitreous changes: Group 1 eyes with a posterior vitreous detachment (PVD); group 2 eyes with a posterior vitreous schisis (PVS). RD and MH examined by optical coherence tomography (OCT) showed different features in the 2 groups. Surgical techniques were chosen on the basis of clinical features of RD.

Results: The postoperative recovery of the visual acuity was higher in eyes with PVD (mean 0.33 ± 0.12) in comparison with eyes with PVS ( mean 0.24 ± 0.14).

Conclusions: In this study vitreous changes appeared to be the ground for the features of RD with MH and conditioned the choice and the results of the surgical treatment.
